The Sports Medicine staff here at Campo Verde High School is dedicated to ensuring a healthy and safe atmosphere for your athletes. Through the use of Sportsware software, we are able to include all athletes/patients health information, with baseline concussion testing in one place for ease of access. The software allows authorized personnel, athletic trainers, team physical therapists, and physicians to incorporate a state of the art environment for the safety and treatment of patients. Please help us by completing the physical packet information, then putting the information into the Sportsware system.

Follow the directions (after completing your physical packet):   Sportsware Directions

*If you have any questions, please contact David Mesman at:  david.mesman@gilbertschools.net
