Concussion Course - Complete with Athletic Packet
Each athlete must take the MTBI/Concussion Course and submit the certificate with his/her athletic packet before he/she can be cleared for a sport. This certificate will only need to be done one time in the athlete's high school career. This course can be found at:

GPS Athletic Fee per Sport: $100
Uniforms will not be issued until fee is paid.  Athletic fee may be paid with tax credit donations.  There may be additional fees required per sport, please contact your coach.

Announcing our Sports Physicals Day!

As part of our ongoing effort to provide collegiate level care for high school athletes in the East Valley, Mezona Orthopaedic invites you to our Sports Physical Day to be held on April 25th.  Join your teammates at our Rome Towers location and get your sports physical completed.  

The cost of the physical is $25 (cash only) and sub specialty physicians will be on hand to assess and provide consultation as needed including cardiology, neurology and orthopedics.

If you are plan on attending the event please have a parent/guardian with you or complete the consent attached.  If addition, you must have the AIA physical form completed prior to your visit.  

We have assigned each school a specific time in order to better serve our patients.

Campo Verde's assigned time is 7:00 AM - 8:30 AM.

Stadium Policies: NO outside drinks allowed in the stadium, only unopened bottle water. There is NO re-entry, even if you have an activity card, and you MUST show your student ID at the ticket window to receive the student price.


Rules and Regulations for Gilbert Public Schools Athletics
State and school academic requirements: The Extracurricular Activity Eligibility Policy, JJJ, shall apply to all students who choose to participate in the District's interscholastic programs.
  1. Students must meet the state requirements of passing all classers. An "F" grade. An "I" or a "U" grade will constitute a failure to pass a course.
  2. Students must meet the District requirement of a 2.0 G.P.A. carry over from the previous grading period.
  3. Students must have a physical examination completed per the Gilbert Public Schools Athletic Clearance Packet.
  4. NCAA Clearing House Eligibility. For information refer to the Gilbert Public Schools Course Description Book
