Requests for High School Student Transcripts
You can securely obtain official transcripts for a GPS high school student only through Parchment. Please visit your high school website or Parchment through the links below to get started.
GPS Public Records Requests (FOIA)
This request form is intended for requests made by members of the public for information requested under the Freedom of Information Act (FOIA).
Evaluation Copy Requests
Please fill out this form to request a copy of any archived GPS evaluations. We have evaluations archived from SY16-17 forward.
Please note evaluations were not completed for SY20-21 as per Senate Bill 1165 passed by Governor Ducey due to COVID.
We will only provide the year end summative evaluation unless you have an additional request.
Please allow ten to 14 days for processing.
Special Education Records Requests
Requests from parents/students/legal guardians must include: Student’s full name, date of birth, a completed release form, and identification of requestor verified.
Requests from other educational agencies must include the student’s full name, date of birth, and a release form with parent and/or school official’s signature.

All requests for special education records will be processed in the order received. Every effort will be made to expedite all requests, but delays may occasionally occur. Please allow up to 10 school days for processing.
Note: All Special Education Records (including placement records, referrals, evaluations, program plans, testing data, etc.) will be destroyed four years after the fiscal year of final program enrollment.
Retention of Special Education Records
In accordance with ARS §41-1351, and the General Records Retention Schedule for School Districts and Charter Schools Student Records (#000-11-53) established by Arizona State Library, Archives and Public Records (L.A.P.R.), the retention period for Special Education Records is four (4) years after the fiscal year of final enrollment.. Gilbert Public Schools will destroy Special Education Records of students who have graduated, been dismissed or withdrawn for four (4) years or more from the current academic year.
Prior to the destruction of records, you have the right to review the records and obtain copies of any information. To obtain records, you must be the parent or the legal guardian of the student or the student (age 18 or older). You will receive a notice prior to the records being destroyed, advising you as to who to contact to request copies.
If you do not contact the district by the deadline, all records four (4) years older than the current academic year will be destroyed.
